Funded by the Sacramento County Division of Behavioral Health Services, the Hope Team provides mental health services, supported housing, intensive case management services, substance abuse recovery support and life skills training for adults who have a psychiatric disability and are experiencing homelessness or are at risk of homelessness. These services are aimed at improving quality of life and preventing the individuals from repeatedly cycling through the system. The staff also includes a psychiatrist and a nurse to provide quick access to mental health services and this program works closely with The Guest House, a division of El Hogar.

  • Medication Assessment and Monitoring
  • Substance Abuse treatment
  • Assistance with Medical Care
  • Skills Building Activities

Access to our New Direction case management programs goes through
The Guest House, the mental health clinic for homeless individuals with a psychiatric disability.  Guest House is located at 1400 North A Street, Bldg. A, Sacramento, 95811.
